Terms and Conditions

Mediway — Terms of Service for Free Health Check-Up (Sign-Up)

Last updated: 26 September 2025 (SGT)

These Terms of Service (“Terms”) govern your registration for and participation in the Mediway Free Health Check-Up (the “Programme”) and your use of Mediway’s websites, pages, forms, communications, and related services (collectively, the “Services”). By visiting this page, reviewing these Terms, submitting any personal details to Mediway (including through a web form, QR code, landing page, email, or in-person registration), or attending a Programme session, you acknowledge that you have read and understood these Terms and agree to be bound by them, together with Mediway’s Privacy Notice and the applicable Partner notices in Annex B/C.

If you do not agree, please do not submit your details and do not attend.

1. Provider, Scope & Roles

1.1 Provider. The Programme is provided by Mediway. Where applicable, screenings are performed by Mediway personnel and/or Mediway-appointed healthcare providers (“Healthcare Partners”).

1.2 Operational host (low-salience partner). Sessions may include a logistics host from a Licensed Financial Advisory Partner (the “FA Partner”) to assist with event flow, safety briefings, and optional post-session follow-ups. The current FA Partner is named only in Annex B.

1.3 Independence. The FA Partner and its representatives are independent of Mediway and are not Mediway’s employees or agents. Any financial advisory engagement between you and the FA Partner (if you choose to pursue it) is governed by the FA Partner’s terms and regulatory requirements.

1.4 No emergency care. The Programme is not an emergency service. In an emergency, call 995 (Singapore) or your local emergency number immediately.

2. Eligibility, Registration & Identity

2.1 Age/authority. You must be 18 years or older, or have parental/guardian authority to register a minor.

2.2 Accuracy. You confirm all information you provide is true, complete, and current, and you will update it as needed.

2.3 Identity checks. Mediway may implement identity verification (e.g., NRIC/FIN/passport/Singpass; selfie/liveness) for safety, fraud prevention, or legal compliance.

3. Programme Content & Limitations

3.1 What’s included. Indicative screenings (e.g., vitals, body composition, lifestyle assessment) and other non-diagnostic tests specified on the booking page or at the venue.

3.2 What’s not included. Results are screening indicators only—they do not constitute medical diagnosis or treatment. Always consult a qualified clinician regarding any condition or result.

3.3 Referrals. At your option, Mediway may propose referrals to Healthcare Partners or your own doctor. Any follow-on services or costs are outside this Programme.

4. Disclaimers (Health & Financial)

4.1 Health information. Unless expressly provided by a licensed clinician acting within their professional scope, information shared during the Programme is general wellness education.

4.2 Financial information. The FA Partner’s host may share general frameworks related to wellness financing. Formal financial advisory recommendations are only provided if you later choose to engage the FA Partner in its licensed capacity and after the required fact-find/suitability process.

5. Fees & Changes

5.1 Free screening. The Programme is free for eligible registrants.

5.2 Operational changes. Mediway may modify, reschedule, or cancel sessions for operational, capacity, or safety reasons. We will endeavour to notify you using your provided contact details.

6. Safety & Conduct

6.1 Follow instructions. You agree to follow safety and screening instructions from Mediway staff/Healthcare Partners/venue personnel.

6.2 Fitness to screen. If you are pregnant, have implants/devices, or specific conditions, inform staff; some tests may be unsuitable.

6.3 Right to refuse/stop. Mediway may refuse or stop a screening to protect participant safety or Programme integrity.

7. Personal Data (PDPA) — Collection, Use, Disclosure

7.1 Purposes. To organise and deliver the Programme, Mediway collects, uses, and discloses personal data you provide (including contact details and any health data you submit) for:

Registration, scheduling, capacity management, venue operations;

Conducting screenings; preparing and delivering your results to you;

Follow-up communications related to your screening (clarifications, reminders, post-session checks, navigation to appropriate care);

Safety, incident reporting, quality assurance;

Identity verification, fraud prevention, audit/compliance;

Analytics/service improvement (including de-identification/pseudonymisation); and

Creation of legally valid consent and attendance audit trails (e.g., timestamp, IP/device, channel logs).

7.2 Disclosures. For the purposes above, Mediway may disclose relevant data to:

(a) Healthcare Partners engaged to conduct screenings;

(b) The FA Partner (defined in Annex B) for Programme-related follow-ups and care navigation;

(c) Contracted technology/communications vendors (which may process data outside Singapore with PDPA-compliant safeguards); and

(d) Regulators/authorities where required by law.

7.3 Security & retention. Mediway implements reasonable administrative, technical, and organisational safeguards and retains data only as necessary for Programme and legal purposes.

7.4 Your rights. You may request access/correction and withdraw consent at any time per the methods described in Annex B (or by using the unsubscribe/STOP options within communications). Withdrawal may affect our ability to provide Programme services (e.g., scheduling or results communications).

8. Contact & Telemarketing Consents (PDPA / DNC)

By submitting any personal details to Mediway (including via form, email, QR landing page, or in-person registration) or by attending a Programme session, you expressly give clear and unambiguous consent (in evidential form) to the following, until you withdraw under §8.4:

8.1 Programme service communications (required). You consent to Mediway and the FA Partner contacting you for Programme-related service/administrative purposes (e.g., scheduling, reminders, results clarification, safety/quality checks, and care navigation) via:

Voice calls (mobile/landline/VOIP),

SMS/MMS, WhatsApp/other chat apps, push/in-app messages,

Email and post, and

Automated reminder/alert systems where appropriate.

Notwithstanding any registration with the Singapore Do Not Call (DNC) Registry, you consent to be contacted via the channels above for the Programme and related service/administrative purposes.

8.2 Financial-advisory follow-up (express consent to be called). You consent to the FA Partner contacting you via the channels in §8.1 to:

discuss screening-related wellness/coverage queries,

provide financial-advisory information, and

arrange a follow-up appointment to conduct any required fact-find and suitability assessment if you wish to explore solutions.

This consent applies even if your number is in the DNC Registry and remains effective until withdrawn.

8.3 Evidence of consent. You agree that your submission/attendance, together with system logs (timestamp, IP/device, source page, and referrer), constitutes evidence of express consent for PDPA/DNC purposes.

8.4 Withdrawal of consent. You may withdraw consent at any time (no fee) via the methods provided in Annex B, or by using “unsubscribe/STOP” instructions in communications. Withdrawal does not affect prior processing and may limit our ability to deliver Programme services.

9. Technology, E-Records & Third-Party Tools

9.1 E-records. You agree that electronic records (including click-throughs, form submissions, server logs, and communications logs) are valid evidence of your agreement and consents.

9.2 Automations. Mediway may use automated messaging for reminders/alerts.

9.3 Third-party tools. Certain booking, identity, or messaging features are provided by third-party vendors under their own terms and privacy policies.

10. Intellectual Property

All Programme materials, content, and branding belong to Mediway or its licensors. You receive a limited, personal, non-transferable licence to use materials provided to you for private, non-commercial purposes.

11. Indemnities & Limitations

11.1 Indemnity. You agree to indemnify Mediway, its officers and personnel, and the FA Partner’s host personnel from losses arising from your breach of these Terms, your misstatements, misuse of the Programme, or unlawful acts.

11.2 Limitation of liability. To the fullest extent permitted by law, Mediway’s total aggregate liability arising out of or in connection with the Programme and these Terms is capped at SGD 200, excluding liability that cannot be limited under law.

12. Changes to These Terms

Mediway may update these Terms from time to time. The effective date appears at the top of this page. Where required, Mediway will provide additional notice. Continued submission of details or attendance after an update constitutes acceptance of the updated Terms.

13. Governing Law & Jurisdiction

These Terms are governed by the laws of Singapore. You agree to the exclusive jurisdiction of the Singapore courts.
